Is there a particular form that people use? I take readings 3 times a day, save the weekly reports, listen to the alerts that come in as they are emailed to me.
I just want to be in as compliance as possible.

If I walked in to inspect, I would have ZERO problems with that, provided the logs are signed and dated showing evidence of review as required.
I really like to see things like required EAS RWT/RMT entries circled or highlighted. That tells me the logs were really reviewed. HOWEVER if the Chief Operator shows me that everything is really present on that signed/dated log with no highlights or other callouts for required entries I'm fine with that.
