When I first started working here, I discovered that an ‘engineer’ here had used Belden 8451 for a 120v application.  His comment was, “well, the wire says it has a 400v breakdown voltage” which was more than 120v rms.  Oh boy.

Disclaimer: no one was injured in this story.

I would be embarrassed to tell most of my embarrassing stories on this forum...

 

I will, however, tell one on one of my early assistants...  

 

While cleaning out a bunch of old wiring from under a console, he cut what was supposed to be a dead power wire to an On-Air light.  It wasn't dead.  His first words to the on-air guy as the turntable ground to a halt were "disregard that odd burning smell".  He didn't seem to notice that the room was dark and silent, nor that he had a small mushroom cloud rolling up around his head...

 

He had checked the wire for power with the mic on but somebody had kludged the SO cord to power the light w/ mic on and something else (later determined to be a fan) in the off position thru a DPDT relay and SO ground wire.

 

This was long before the non-contact voltage detectors became cheaply available.
